Who We Are

Universal Personalised Care (UPC) is an organisation offering services to people living with disabilities who are funded by the National Disability Insurance Scheme (NDIS) We work with people living in Brisbane and surrounding areas who are Self and /or Plan Managed under the scheme. UPC supports people with physical disabilities and those with psychosocial disabilities that are due to severe and complex mental health illnesses including forensics and those transitioning from correctional centres/prisons, and in-home care to a wider population from children to older adults. We also provide Community Nursing services.

UPC is currently going through registration to register as a Provider with the NDIS Commissioner. They have recently completed their auditing process and are now waiting for the NDIS Commissioner’s decision in the next few months to learn of the outcome.

Aims & Objectives

Our aim is to deliver the Universal Personalised Care (UPC) Standard for our local communities.

Our Objectives are to ensure that:

People are more involved in decisions about their health and care with the persons voice at the centre co-producing plans.

People who live with long-term conditions feel supported to develop the knowledge, skills and confidence to manage their own health, care and wellbeing as independently as is possible.

People are aware of their choice and control rights and have the information they need to make informed decisions about their care and how it is delivered.

People are given options, guidance and support to inform their own care plan and how to access help when they need it.

People’s individual assets, needs and preferences are personalised in a way that matters to them.

People’s care is personalised through decisions based on individual strengths and needs.

Our mission is to bring a fundamental shift in how we work alongside the individuals, families and communities recognising that the importance of ‘what matters to someone’ is not just ‘what’s the matter with someone’.

We encourage people to grow, develop and gain strength and confidence in addition to supporting them to make independent decisions, take control of their life and exercise their rights. We are guided by participants, their families/carers rather than imposing.
